To receive your travel allowance for your medical travel, fill out, sign and return to us the mileage forms (that we gave you).
Caution: This rule applies to Orange, Osceola, Seminole, Lake, Volusia, Marion, Sumter, Hernando, Citrus, Putnam, Flagler and St. Johns counties. This rule does not apply to Dade, Broward, Palm Beach or Monroe counties. How this rule applies in other Florida counties has not been determined by the courts.
Think of this benefit as "gas money". Each time you travel to (and from) your doctor, pharmacy, MRI center, or to physical therapy, etc., write down the number of miles that you travelled (for medical reasons only) on one of the mileage forms (that we gave you). When you want to be reimbursed (paid) for those miles (sometimes called "mileage" or "travel allowance"), sign the mileage ledger form (that we gave you) upon which you have written down the number of miles you have travelled (for medical reasons only) and return it to us, so we can send it by certified mail to the appropriate auto insurance company. Be accurate. The insurance company will verify (check) that you actually went to the doctor or pharmacy, etc. on each day you claim you did (by cross-referencing the doctor's records or the pharmacy's (drug stores) records with your mileage form). The insurance company will not pay you the actual (real) costs of your medical travel and is not required to pay for, or provide you with "transportation". Instead, the insurance company will pay you an arbitrary amount per mile (no matter what the actual costs or your medical travel or "transportation" really were). The insurance company will not advance you money up front to spend for medical travel. The insurance company will not pay you for your mileage allowance for (up to) thirty (30) days after they receive the mileage form (that we gave you) correctly filled out and signed by you. So do not delay. If (like most people) you have a $1,000.00 deductible that applies to your medical bill, wage loss, mileage, or yard and house work claims, you will not receive any mileage allowance for several weeks, because it takes that long for 80% of your medical expenses to exceed the $1,000.00 deductible amount. Once that happens, you usually get paid your mileage allowance right away. Your insurance rates will not go up because of your mileage claim (unless you were at fault). Florida Law # 626.9541 (o)(3)(4)(5). We do not charge you a fee for this. Our fees are based on the amount of any settlement later in the case.
